John Wayne's main termal building, divided into three concourses, is accessible via the I-405, SR-55, and SR-73 freeways. There is limited bus service by OCTA and Irvine Shuttle.

Service is primarily domestic, but the airport has received a small number of pre-cleared flights from Canada over the years. A Federal Inspection Services station opened in 2011, allowing several airlines to operate to and from Mexico.

The airport is known for its strict noise regulations. A weekday curfew applies between 10pm and 7am during which scheduled flights are prohibited. Takeoffs often involve a steep initial climb at full power followed by a sharp throttling back of the engines while overflying the City of Newport Beach, or a sharp turn very soon after takeoff to avoid the same.
