Looking at a random Wednesday morning, I'm seeing roughly one fast train every 30 minutes from the centre of Barcelona to Tarragona, along with slow ones!. All you'd need to do is take the bus or metro into the city to catch it

Would this be a Domestic or International flight? US Airways don't seem to offer First class on their long-haul international routes, only Business class, but there's quite a difference between the domestic first and international business seats (with the international business ones being better!)

Many airports, including western airports, offer this sort of service for a hefty fee. This is the page for the Heathrow Airport VIP service. I would suspect that this company simply delegates the airside parts to the appropriate provider at the airport, then arranges for their driver to be ready to accept you once to come landside. Be aware that it will not be cheap!

I'm not disputing that, which is certainly the default case, but if you look at the linked answer you'll see that there are exemptions from those normal rules for people holding visas from certain countries

If you have an anytime or offpeak or super offpeak ticket, then the seat reservation is optional. Advanced Tickets are only valid on the booked train, see the national rail website for details

The seat reservation bit isn't quite right - if you have an Advanced Purchase ticket like I do, without the seat reservation there's no way to prove what train you are booking onto, so the ticket alone wouldn't be valid!
