Mixed gender onsen are indeed increasingly rare, and younger ladies frequenting them tend to wear swimsuits these days. Most of the remaining ones I know of are public, free and have no staff on site. They're also very basic, with usually just a single bath, so they're mixed more by necessity than choice -- there isn't really any alternative!

One would definitely have to look at the statutes state by state. In the state of Florida for example you could be cited under the public indecency statute. In NY though going topless is legal for women, so partial nudity would be fine in that state.

Lady Bay is a part of the Sydney Harbour National Park.
In general, Sydney Harbour National Park is only open sunrise to sunset. No, there are no nudist hostels, but you can easily find some nudist campsites in Europe or even some nudist hotels. Note that some naturist campsites only allow you to camp there if you are a couple to prevent voyeurs.

It turned out that while there were separate areas for men and women for the indoor spas, outdoors, there was only a mixed section, and a section for women only. So being in the mixed section was not so much permitted as mandatory!
